[bug-mailutils] Documentation for MH

I plan to go through the MH specification this weekend and plan to make a very 
rough outline draft for use in the Mailutils documentation.

I will use all of the README's in the mailutils distribution for reference.

I am going on the assumption that mailutil's MH is mainly for mh-e.
This is what I use it for so my outline will be mh-e oriented regarding the use 
of mailutils' MH programs.  If I am wrong here please let me know.

I will send my very rough outline for MH documentation next week, so that you 
can all review and respond to it..
