creating primitives

For the life of me I can't figure out how to create primitives and the 
associated gate entities for them.  What I want to do is create a set of 
objects that I wish to write custom net-als files for.  I have tried 
creating the net-als facet for one, then I created an icon facet of the 
same name and containing exports matching the ports in the net-als 
file.  However the net-als version of my test schematic in which I use the 
icon I created does not contain the icon's net-als code.

Am I going about this the wrong way?  Ultimately I would like to custom 
create an instance I can place in a schematic that looks like I want and 
uses gate net-als code of my creation.  Someone please embarrass me and 
tell me where in the manual I can find an explanation of this since I 
cannot find it myself.
