[Gnump3d-users] forms

My access log file is showing that some files are giving 404 connection errors. 
 This is strange since I can find them on and off my network.  My
server is now wireless, so maybe it's getting some interference.  I'm wondering 
if other people are getting errors and would like to put a form on my
page so that I can get feedback.  I see that there's already a form feature 
setup in bug.html to contact Steve.  How could I put together another
form in gnump3d that would put the form feedback on my server?
