Sorry I don't quite understand your point. 

GNU Health allows you to easily search for a person / patient entering
any of her / his IDs on the tree view. It automatically scans through
the PUID and alternatives IDs without the need of specifying them. 
So, in your case, you can enter the legacy Medical record or the
current ID, and the patient will be found.

But again, I think I am not understanding your point.
