* --header and --footer options added by Xavier Outhier

beginning and at the end of the generated document. 
* GNU gengetopt is used for command line parsing. 
* hexadecimal and floating point numbers are treated (Tom Hull

* fixed the bug of /* comment in another environment (notified by
Michael Gasche <address@hidden>)
