cppcheck identified a few places where the scope of a variable may be  
reduced.

[lily/accidental-engraver.cc:54]: (warning) Member  
variable 'Accidental_entry::origin_engraver_' is not initialized in the  
constructor.
[lily/ledger-line-spanner.cc:148]: (warning) Member  
variable 'Ledger_request::excentric_' is not initialized in the constructor.
