Indexes with report

I'm using the report style and I need to generate an index.  The index
is generated as expected and starts on a new page.  The headers say
Index appropriately.  However, there is no section heading introducing
the index like this with the book style.

I read the code in reportf and bookf, but I can't see where the latter
sets the index heading.  Any suggestions?
