This does indeed sound like a bug.  I gather that the problem
is only that there are headers on those intro pages that are
otherwise intentionally left blank.

In Dave's particular case it seems that all the intro
sections occupy only one page.  A workaround in that case
would be to set the intro even pages headers and footers
to @Null.  But that won't work in general.

I'll take a look at it and see what I can come up with for
the next release.  There may be a problem, because intro
pages are generated two at a time (to ensure there is always
an even number of them), so their implementation is different
to ordinary pages.

Another problem is that some people might actually want those
extra headers, but I suppose the majority don't.
