Al,

I had forgotten to mention that the index is my "TOC."
If you substitute "index" for "TOC" in my original email, it does make
more sense.

I want:
TOC
A - M
N - Z

and lout produces:
A - M
N - Z
index

Then I move index to TOC by perl script.
