You could try something like this (untested):

@RunningOddTop{
{Nombre: _________________________ @Right
Fecha:______________}
//1f # you'll almost certainly need to adjust the 1f
{This is on a second line; you'll need to make sure your top
margin is tall enough to allow for it}
}


I use a similar technique to get a ruler line under my page headers.
