the problem is in the server. A HEAD for the page gives back:

   HEAD /class/ps142k/WWW/casebook/casebook.htm HTTP/1.0
   HTTP/1.0 200 Document follows
 
   Date

   Server

   Content-type
          text/plain

They probably did not put .htm as a valid suffix in the server's mimtype
file...
