lynx-dev correct </form> syntax in options menu

While looking into trace mode I found out that form-based options menu
have several HTML errors:

1) <label> not allowed inside <pre> </pre>     --just ommit <label></label>
2) </option> not valid at all within <select > </select>  --ommit </option>
3) </form> was not closed before </body>.                       --insert it
