I got the same display with 2.8.3dev.6 (time to update).

I don't know much about forms and what not, and it may not have
anything to do with the display anomaly, but I'm betting that
interleaving form and table tags as they do is not a Good Thing.

The text looked pretty normal in source view -- would trailing
spaces make any difference?  Every line seemed to have one when I
threw the text into an editor, but I can't see that having an
effect.
