Summary: fix(?) the problems with hidden unread items etc. also add loads and 
loads of debug spew

Hopefully fix several problems:
        - no more hidden unread items
        - no more loading of duplicate entries from the db
        - no more problems with wrong order of the items

I have given it only limited testing, though. There's still a metric shitload 
of debug spew, which
probably helps no-one but me and even that is questionable.
