I am no BBDB expert, but I believe I can answer two of your questions.

Does `C-u :' do what you want here? If so, consider customizing
`bbdb-mua-update-interactive-p'.
Here its value is `(search . query)' (which I think is probably the
default), and it sounds like you want `(query . query)' or even `(query
. create)'.

Perhaps setting `bbdb-mua-auto-update-p' to `search' is what you want
here.
