[ccd2cue web pages] Proposed changes

Hello,

We are planning to migrate all templated pages on gnu.org to HTML5 in
order to improve accessibility. But before doing this, deprecated tags
such as <tt>, <acronym> and <big> need to be removed.

The web pages of ccd2cue contain <acronym>. I got rid of it, and took
this opportunity to make other minor changes and modernize the markup,
as follows:

* Replace <acronym> with <abbr>.

* http > https where possible.

* Remove "https://www.gnu.org"; from local URLs.

* Fix a typo in the Manifesto.

* Add brackets around email addresses.

* Replace name attributes with id.

* Use <kbd> for commands throughout (special styling).

* Move page headings (News, Acknowledgments, Thanks, etc.) to the main
heading to save space on smartphones.

* Replace tables with a more flexible layout (classes "emph-box" and
"toc-inline" are defined in layout.css).

* Update to boilerplate 1.92 and reduce line width.


A diff is attached. If you agree with the changes but don't have time
to commit them right now, I will gladly do it.
