task list item creation_time

Hi,

  * Add a timestamp for task and thread creation to the relevant info
  structures (and make sure it works for the kernel task and threads).

Thomas added this for threads in 1997, Roland for tasks in 5/99.  It is
available in libps for tasks (with %start).  Is there anything else that
needs to be done for this item?  Verifying that it works for kernel threads?

If not, I suggest it can be removed.
