incorrect positioning of accidentals

Thanks.  I sligthly revised the algorithm, it now always puts the top
one on the right.

Kludged in a correction for this case.

Ok, fixed.

I also found two deficiencies in the code, but finding those is left
to the reader as an excercise. :)
