Yes, this is a long-standing bug, which occurs when you don't
specify any absolute dynamics before having a crescendo/diminuendo.
The problem is that LilyPond doesn't set any default volume to
start with. Actually, if you have a score without any dynamics
at all, the MIDI file will not contain any dynamics settings
either, so you get the default for the MIDI device you use.

The following score causes Lilypond 2.3.11 to create a MIDI file where
only the first note is audible (yes, the fourth C is gone too):
\score {
\relative c' { c4 c-\< c-\! c }
\midi { \tempo 4 = 120 }
}
If you remove the crescendo, the MIDI file is OK. Also, if you put a
"-\f" after the last note, this become audible. I think Lilypond
creates a MIDI file where all the notes but the first have zero velocity
(volume).

Note: This bugreport was already sent some time ago
(http://lists.gnu.org/archive/html/bug-lilypond/2004-08/msg00179.html),
but nobody confirmed if this is a Lily's bug or if there is something
wrong with my hardware (Debian Sarge Linux on an AMD PentumIII machine
with Timidity).
