midi & pdf disagree with conflicting tempo-changes

Hey guys,

When simultaneous tempo changes disagree:

1) a message reports "junking" all but the first one found;
2) and the first one found is printed in the pdf;
3) but the *last* one found is used for the midi output.

This is inconsistent and should be fixed!

Here's a small file demonstrating the problem:
