Thanks.  Many of these are already fixed in the development version, but I 
have applied those of your suggestions which weren't.

I think this is fine as it stands.  For example:

  changequote(foo, bar)
  changequote([)

will eventually set the close quote character to "'" (the default).

Yes please.  The 1.4 manual is very old, and in some places quite out of 
date.  The current texinfo manual uses <address@hidden>.
