[PATCH 0/8] DWB/Heirloom support and minor fixes

In this series, I've enabled/verified passable output (to terminals)
from DWB 3.3 troff and its descendant, Heirloom Doctools troff.

Complex tables do not render spectacularly with these formatters
(terminal output in particular was always a red-headed stepchild for
them), but by getting the ncurses man pages working with DWB 3.3 troff,
I think we can have more confidence that they will work with surviving
System V troff deployments.
