Just on this minor point: they're not.  makeinfo warns about them.
(Seems like it could jus as well be an error.)
nestref.tex:7: warning: @ref should not appear in @xref
nestref.tex:7: warning: @pxref should not appear in @xref
..

For the line:
@xref{Top, @ref{Top}, @pxref{Top}}.
