Nothing that I can see.

One thing that I'd put it down to is that smsd communicates to mysql
over a WAN/VPN, even though smsd doesn't complain that mysql has
disappeared for outbox reads.

Is smsd threaded? Could the inbox thread get stuck but the outbox thread
be able to recover from any problems?
