It is an important old book. I believe it is out of print, and has been
for a long time. It has good coverage of the state variable method, and
others that have been mostly forgotten.

I don't know. I think Berkeley does their own publishing. Many U.S.
universities use "University Microfilms" to publish their
dissertations. You might try contacting them. Try google.
