Hi all,

Just revised the form to add a new person user. Some notes:

• UPPERCASE (or Camel Case) headings can be achieved with CSS text-transform
• Moved "middle name" field between first name and last name :)
• Removed class="notRequired"; we can just use it as the default.
• We shouldn't use _javascript_ which relies on mouse events. The attributes used are not even valid HTML 4.01.
  
• You can use white-space: nowrap; instead of  s to avoid breaking text into several lines.
• Valid XHTML 1.1.
