* patient data fetched in 2.252 seconds

Note:
- same old machine (133 MHz/40 MB)
- same search term ("becker")

But:
- make view v_basic_person use indices on BOTH names and
  identity correctly
- optimize queries to actually use indices in the most common
  cases for search terms (i.e. try smart case-sensitive first)
- no parallel import either, but that did not consistently
  have a large influence - it much depended on the actual
  swap state/IO contention of the machine

Now, THIS is where we want to be !
