Much of the complexity is quite hidden and lies in a
well-thought out model for the data. If doctors couldn't care
less about all this episode/health issues/encounter crap they
can just ignore it (so everything gets attributed to the
__default__ episode/issue/encounter, use the single box SOAP
widget and be done with it (and everything will still work albeit
redundant data entry will necessitate itself as you noticed in
OSCAR).

It is. Actually, something better than CVS but that isn't
available under a reasonable license, I suppose.
