should there be domain tables for clinical actions ?
e.g. 
  clin_root_item
    --  clin_action
         -----  clin_prescription
         -----  clin_instruction
         ------ clin_measurement
         ------ test_order
         ------ referral
         ------ vaccination
         ------ form_filled

as an example inheritance scheme ?
