Excellent data point. In view of lack of other evidence I have
set the default for the soft timeout (only below which all
encounter possibilities are automatically regarded as
continuations). If one wanted to maximize ROB (return on
billing) one could set the hard timeout to 4 hours as well,
meaning GnuMed would always create a new encounter after a 4
hour gap...
