Ok, I still don't know why but at least I got it to work.
I always tried with workplace *any* which did not exist yet.
Therefore there was no plugin load order.

Going back to plugintest4 which I had used before now works as expected.

I am wondering why starting Gnumed with a new workplace name sets up this 
workplace in the database while standalone use does not.
