[Gnumed-devel] Update of PatientPhoto stuff

I've moved the import/aquire menu to popup over the patient photo on the main 
title bar. I've changed the type of dialog from a file-load dialog to an 
imageload dialog which is more visual. Someone will have to implement the 
internal code for saving/changing the picture.

I've also removed the patient photo from gmDemographics as previously agreed 
upon.
