Patient Request(s) describes the "agenda items" when an agenda is set 
purely by the patient.

But sometimes the visit is for a purpose NOT initiated by the patient.

So it could be "Purpose" (since "Objective" would need disambiguation 
from the Objective examination).

To all, within Purpose (or Patient requests or RFE), does it matter 
whether one part "followup high BP" was doctor-initiated while some 
other part was patient initiated?
