[Gnumed-devel] wiki formatting note, and a change to what was labeled "Other"

To prevent a mixed case word (e.g. HbA1c) being rendered as a 
WikiWord, you used to have to pre-pend <nop> (e.g. <nop>HbA1c) but 
now you can instead use an exclamation mark (!HbA1c or !GnuMed)

I have renamed, and moved,  {Pathology Tests, Social History, Medical 
History}  to:
