on the question of how names are assembled, presented and searched in the search field, is anyone strongly wedded to the current "formula":

        title given_names surname

I had thought that

        surname, [title, if present] given_names

is common if not universal but perhaps I am wrong. One advantage is that it is clear where the surname ends and where the given_names begin. It is possible for patients to have two-part first names, a middle_name (or sometimes a third given_name) and/or a two part surname that is not necessarily hyphenated. Therefore when you see

        Mr Peter Avery Smith

you can't tell whether the surname is Avery Smith or whether it is just Smith.
