[Gnumed-devel] default state added

I have added a default state to every country that does not
have any states associated with it (and only those).

This is self-healing in that the function
gm_upd_default_states() can be run any time thereby ensuring
only countries without a real state have a default state.

The bootstrapping/upgrading scripts take care of this.
