I was only thinking the text or text-like objects. Within a patient the use case could be a diagnostic consideration that got mentioned in a report but which was not at the time given serious consideration and/or was missed and therefore not coded as meta-data.

The across-patient (practice review) use case could be to find special patients, again based on content that had not been coded.

I confess I lost sight of whether some incoming data --- reports that might have been sent through secure messaging rather than faxed --- will be going into some other storage area that better lent itself to searching... will it be going into a different storage area in the schema?
