I am trying to run the client , using an older database.
Is there a way of knowing which version database is running ?
Also , how to upgrade from one version to another ; apparently this can
be done non-destructive of data ?

With customized clients, presumably use of the backend is preferred. 
How is it best to add a hook into the demographics search use case, 
so that an alternative database for demographics is searched and
displayed,
 and then when one is chosen, an import is made from the alternate
database, prior to opening the record in gnumed.
Is the idea of destroying the record in the source database to preserve
one-copy consistency ? Perhaps this can be optional, because there are
probably more users just wanting to evaluate another medical record
system, but not necessarily give up their old one . 
        So back import of updated record into the originating database would be
a good feature, because it widens the number of potential users. ( e.g.
in a clinic where policy is very conservative, to continue using what's
known, but there is leeway to use any other
system that integrates with the original system ).
This might be a difficult thing to do well though, because not only 
correctly formatted inserts into tables have to be done, but also
indexes may need to be updated. 
In debian , there is a libxdb1, which might be the way to go wrt to
updating indexes , but it would have to run in cygwin .
