Well, the easy steps are:

1) ignore health issues
2) encounter is the consultation
3) episodes are the illnesses you work on during consultations

Health issues are things like "Diabetes type II" (the
patient always has it) while episodes are things like
"common cold" (the patient only has it for a while).

Tell me your speciality and I will give you an appropriate
example.
