Indeed, I see. I sometimes had a gut feeling on this like
"why does this EMR list that many problems in the stats ?"
but never analyzed it further.

I tend to agree - for the purpose of EMR statistics at
least. Inevitably someone will (mis)use the issues and
episodes for managing things in other ways which would
warrant the above stats behaviour but, oh well, "proper"
use tends to suggest what you show here IMO.
