Regarding the encounter concept:

in GNUmed this is a fusion of

   a database session in technical terms

and

   a participant-health care system interaction in conceptual terms

Most of the time it will involve a patient and a provider. In all
cases it will involve access to the medical record of a patient.

One could also think of it as "one incidence of care" which
happens within an "episode of care".
