[Gnumed-devel] discontinuing substance intake

Attached find a screenshot from the discontinue-drug git
branch.

The [Per plan] button will set the discontinue date to
"started + duration" or "today" (if the former is not
available). It will also clear the reason field.

If the [] Allergy field is checked the reason is set to
"not tolerated: ..." where "..." is whatever is already in
the reason field. It will also create a drug allergy and
invoke the allergy edit area after saving the substance
intake entry :-)
