That would mean the user entered a vaccine with that name.
As far as GNUmed is concerned this is a real vaccine.
The fake ones are auto-generated from the indications and
auto-maintained.

That's right. But that's true for other things as well,
document types, medications, you name it.

Sure. If the vaccination entry is edited at the individual
patient level it is *linked* to vaccines. Thus editing will
not change the vaccine, of course. This is the same as with,
say, drugs or test types.
Only if the *vaccine* itself is edited from the vaccine
management UI will it affect all patients with vaccinations
linked to that vaccine. Which is to be expected :-)

Yes and no, keeping in mind the above cautions.
The schema doesn't allow for NULL or empty strings in that
field. However, no one's stopping anyone from entering "?"
or "unknown" ;-)
