[Gnumed-devel] wanted: template for printing vaccination entry

I am looking for a LaTeX template GNUmed can use to print a
single vaccination entry.

This is to be used for handing a copy to the patient when,
say, a tetanus booster is done when the patient came in with
a laceration. It is not intended to show the vaccination
history. This should print on something like an A6 format
sheet.
