I must say this encounter per OBR business is the one part I dont at all 
like about this hl7 stuff and I dont think it will go in.

Truly, we need to figure out what OBR-OBX *means*, *clinically*. If
they dont mean anything to the clinician - drop it ! If it does mean
something we need to ask ourselves whether we need to model that. It
certainly does NOT mean "I am an encounter". I have a gut feeling it
will be another table, something along the lines of  
"clin.ordered_test_battery_within_one_request" or some such. If we need
that we will of course add it !
