I have implemented patient tags in GNUmed.
Tags are tiny images with a short description that can be
linked to a person (and later searched for). They are
defined site wide (description and image) and a comment can
be added per patient.
Apart from the obvious uses (mark impairments, etc) tags can
be (ab)used to provide a way to store information on people
for which there is no dedicated way of documentation yet,
say, pregnancy.
In the top panel they line up below the name to the right of
the patient picture. Kindly observe the screenshot.
