Thank you for the kind help. When I expand $<progress_notes::ap// {\item %s} \\ >$ to $<progress_notes::soap// {\item %s} \\ >$ , it produces an output in a reverse order. I have attached the output here. How can i correct. Moreover, can I use $<progress_notes::so// {\item %s} \\ >$ to print S/O and $<progress_notes::a// {\item %s} \\ >$ to print A and $<progress_notes::p// {\item %s} \\ >$ to print plan.

Here is what I did:
