Unpack it and poke around.

as a first step you can compile the gnumed-client.nsi file and see what 
happens.

It just occured to me that the instructions are incomplete. You need to 
install nullsoft installer and apply a patch (one time) as well as install two 
optional modules before

I am reworking

but in the mean time you can try to compile the nsi files and report any 
(expected errors).Once you have solved those creating a new exe is a speedy 
thing.
