The GNUmed database schema can now attach any number of any
type of code to either of

        clin.episode
        clin.health_issue
        clin.clin_narrative
                -> each of S.O.A.P., that is
        clin.encounter
                -> not *each* of .RFE/.AOE therein though, unfortunately

Now on to

        - input
        - display
        - data pack download
