Hi Richard,

You wrote

        At consulting rooms
        Case conference
        Email
        Home visit
        Notes without patient present

How do you use them? For example

        Teleconference
        …       ≥ 3 people on the phone (only)
        or      = phone call with a specialist


        Case conference
        …       = ≥ 3 clinicians (only), in person
        or      = other?

        Email
        …       = content of the email
        or      = summary (reference) that the email exists elsewhere

Also do you make your "Nursing home visit" entries at the nursing home e.g. on 
a laptop, or do you input them later?
